Policy
1. I have read and agree to follow the Facilities Use Policy, which is incorporated here by reference.
2. Moving Furniture: I will not move any furniture in the Sanctuary. I will not move any other furniture without the assistance of an SFUMC authorized designee, which I hereby request.
3. Equipment: I understand that I am responsible for any damage to or loss of SFUMC property, equipment or the physical premises during my use. I understand that no SFUMC equipment may leave SFUMC premises. I understand that I will be notified and billed for the repair or replacement of any damaged items, regardless of the cause of such damage or loss. I will notify the SFUMC Business Director at 919-362-1666 x223 immediately upon discovery of any damage or loss, either prior to or during use.
4. Clean-up: I understand that I am responsible for cleaning up the area used and to lock the facility upon departure.
5. Personal Property: I understand that SFUMC and its agents are not responsible for any damage to or loss of any personal property located or stored on the premises regardless of the cause of such damage or loss. I understand that except for its willful and wanton negligence SFUMC and its agents are not responsible for any claims or damages in connection with personal injury or property damage occurring in connection with the event registered here. I agree to indemnify and hold harmless SFUMC and its agent from any and all claims arising from such asserted liability, including court costs and attorneys fees.
6. Responsible Parties: I understand that I am responsible for the conduct of all persons associated with this event.
7. Changes: I understand that cancellations and changes to this Request Form should be provided at least one week prior to the event.
