Saint Francis Music Program 2010/2011 Registration

Note to Parents:  Photographs of children may be published on the Saint Francis web site.  These photographs will be used to illustrate musical groups and events.  Most photos will be large groups where individuals may not be identifiable.  In any case, these photographs WILL NEVER INCLUDE any personally identifiable information about your child.  (Personally identifiable information includes your child's name, address, email address, and phone numbers.)  Your typed name in the Parent Name/Signature field below indicates your permission for use of your child's photograph under these conditions.  This permission is granted for an indefinite time unless otherwise requested.  You may revoke this permission at any time in writing.
